To book an appointment with Diana Antes Studio, please fill out this first time client form.

Once filled out, please allow up to 2 business days for an email response from our team with the next steps to get the right appointment booked for you!

Please note - your appointment will not be accepted without a filled out new client form.

Thank you so much for your interest. Please fill out the form below thoroughly. If you are interested in a haircut, please state "haircut only". 

Please note that there is a $50 NON-REFUNDABLE deposit required to confirm your appointment. This will be applied to your services the day of your appointment. You may reschedule your appointment one time before your deposit is forfeited.

Please note: All services done come with a recommended product list to ensure the integrity of services done to the hair. The integrity of the color cannot be guaranteed to last it's full capability if you, the client, decide to use other products outside of that list or purchase the products from other vendors. By clicking below, you are agreeing that I will not be held responsible for fading, changes to the hair's color, or any unsatisfactory results if using products outside the recommended list and/or purchased from another vendor.
